CARDIAC REHABILITATION AND SECONDARY PREVENTION PROGRAM (CARES)
Cardiac rehabilitation at The Medical City is a customized program of exercise and education. Your doctors may recommend joining a cardiac rehabilitation program to help you return to a better health condition when you’ve had a heart attack or if you have other heart conditions. The goals of cardiac rehabilitation are to help you regain strength, to prevent your condition from getting worse and to reduce your risk of future heart problems. These can add up to a better quality of life.
TMC's Cardiac Rehabilitation Program consists of six essential components:
1. Medical Evaluation (risk factor identification) - A comprehensive medical history and physical examination are conducted to assess your current cardiac status, identify any risk factors for future heart problems, and provide individualized lifestyle advice.
2. Nutritional Counselling – Our dietitians will provide tailored advice to help you identify and modify your dietary habits to reduce your risk of future heart problems.
3. Risk Factors Assessment – Our team will evaluate your risk factors and offer personalized guidance to help you mitigate them. These risk factors include cholesterol, hypertension, smoking, weight management, diabetes, and psychological well-being.
4. Physical Activity Counseling – Our clinicians will help you set realistic goals and provide guidance on how to safely progress your physical activity, such as aerobic exercise or walking.
5. Exercise Training/Testing - Our clinicians will provide you with an exercise program that is tailored to your current fitness level and health status. They may also assess your functional capacity with a cardiopulmonary exercise test.
6. Psychosocial Evaluation/Support – Our team will provide individualized guidance and support to help you cope with the psychosocial aspects of living with heart disease.

During Phase 1, the primary focus is to prevent the detrimental consequences of prolonged bed rest. Physical therapy activities commence within the patient's room, encouraging early engagement in sitting, standing, and walking. Additionally, this phase encompasses initial health education and psychosocial evaluation.
Phase 2 begins 1-2 weeks after discharge. Cardiac rehabilitation sessions, which can be conducted in-person or virtually through tele-rehabilitation, are scheduled three times a week. This approach ensures flexibility and accessibility while maintaining the desired frequency of sessions.

We offer comprehensive cardiac rehabilitation services, focusing on education, counseling, and behavioral support.
1. Orientation to cardiac rehabilitation - This service focuses on the patient’s understanding of their diagnosis and how to manage it.
2. Risk factors for heart disease and lifestyle modification - Our team will evaluate your risk factors and offer personalized guidance to help you mitigate them. These risk factors include cholesterol, hypertension, smoking, weight management, diabetes, and psychological well-being.
3. Anatomy of the Heart - Patients will be educated on the structure and function of the heart as well as its role in health.
4. Psychological Referral - If patients require more specialized psychological interventions, our team will provide referrals to appropriate professionals.
5. Understanding your medications - Our team will provide education on the various types of medication prescribed to treat heart disease, their function, and side effects.
6. Benefits of exercise and the heart - Patients will be taught the benefits of regular exercise, such as improved cardiac function and disease management.
7. Dietary education - Our dietitians will provide tailored advice to help you identify and modify your dietary habits to reduce your risk of future heart problems.
8. Diabetes, Hypertension, and Smoking Cessation - Our team will guide you on how to reduce the risk of these conditions and educate you on ways to manage them.
Phase 3 focuses on the long-term health of the patient and includes follow-up visits with a cardiologist, exercise sessions, psychosocial support, and risk factor management. Our team will work to ensure that patients have all the tools they need to maintain their cardiac health after completing the cardiac rehabilitation program.
The program is managed by a team of cardiologists, physiatrists, psychiatrists, nurses, physical therapists, dietitians, pharmacists, chaplain, and other allied health workers. The Attending Physician remains in charge of the patient's medical and surgical management. A formal referral by the Attending Physician is required for enrollment.
The World Health Organization recommends that Cardiac Rehabilitation services should be made available to all patients with cardiovascular diseases. Cardiac Rehabilitation at The Medical City (TMC) is a holistic and multi-disciplinary approach by which a person with heart disease is restored to and maintained at his optimal physiological, psychological, social, vocational, emotional, and recreational status.
TMC's cardiac rehabilitation is an adjunct to the usual medical care provided by your physician, particularly to those who have suffered from a heart attack and have risk factors like hypertension, heart failure, diabetes, and obesity. It is beneficial especially to patients immediately after operations such as coronary artery bypass graft and/or valvular surgery and in patients after coronary angioplasty and stent implantation.
