About Us
Continuity of Care is defined as a process of holistic healthcare management where the patient and healthcare team are cooperatively involved in ongoing health care supervision towards the goal of high quality and cost-effective medical care. This is where The Medical City’s Continuity of Care Center (CCC) comes in. By providing value added services to patients, the center keeps track of their healthcare needs during the entire patient journey from the time of admission, discharge, follow-ups until such time that healthcare services are no longer needed. It likewise delivers a “seamless service” to all patients with utmost quality to sustain continued care.
Furthermore, care will be provided no matter how far the patient is from the institution and whether in an outpatient, work or home setting. All this will be made possible through integration, coordination and the sharing of information among different providers.
Home Care, on the other hand, is the provision of service and equipment to the patient in the home for the purpose of restoring and maintaining his or her maximal level of comfort, function, and health such as acute illness, long-term health condition, permanent disability, and terminal illness. It is a collaborative effort/ partnership among patient, family, and The Medical City professionals.
Home care is also a component of comprehensive health care whereby health services are provided to individuals and families in their place of residence for the purpose of promoting, maintaining or restoring health, or of maximizing the level of independence, while minimizing the effects of disability and illness.
Lastly, Home Care Services make certain that continuity of care is provided to the patient and the people surrounding them inside their residence.

Services of CCC
After confinement, patients will be followed up by the Continuity of Care Center through the Contact Center and Case Managers. The Center keeps track of their health status and needs by coordinating their concerns to all the stakeholders involved such as home care service, laboratory, physical therapy, and other services applicable to them.
The center can also provide patients the available options for equipment, medications, transportation services, and referrals to Private Duty Nurses (PDNs)/Caregivers.
