The Medical City’s Stoma and Complex Wound Care Clinic was established to cater to the increasing number of patients with stoma and wound problems.
The clinic has two separate rooms – a room for stoma patients and another room for wound patients. An adjacent room is also provided for patients who will enroll in its Home Care Program.
The clinic is staffed by highly competent nurses who specialize and are well-trained in the field of stoma and wound care. The Stoma and Wound Care nurses provide pre-operative counseling to patients who will undergo a colorectal surgery that will require a stoma creation, stoma sitting for patients, and patients and family education on the proper care of the stoma especially in the home setting. The Stoma and Wound Care nurses likewise educate the patients on probable complications that may arise in the future, how to handle such situations, and when to get in touch with their attending physician.
An advanced training course on Stoma and Wound Care nursing will be developed to further enhance the skills and knowledge of the health team.

Patient Care Service – Services on stoma and wound care are planned together with the attending physician to further improve patient care and promote good communication and trust for its clients. The team will be mobilized within the different sections of the hospital and wards for fast assessment of these patients where immediate care can be rendered.
Pre-Operative Counseling – This is done prior to the patient’s surgery, it will also include stoma sitting where the trained nurse will assess and plan the patient’s stoma prior to surgery. Other services include prevention of complications, a thorough explanation on the patient’s operation, important information, and entertaining patient inquiries about the stoma and wound to help lessen the patient’s anxiety and assure cooperation about the care.
Post Operative Counseling – This will educate the patients on the proper care of their stoma and wound problems, the proper changing of their Ostomy appliance, wound dressing changes and other health tips as deemed necessary. Other services include educating the patient about the different stoma and wound complications that may arise in the future and how to take care of them in the absence of a health care personnel, where to contact a qualified person in case a problem arises and providing them with important information about the different appliances, products and medications that they can use for their stoma and wound, and allowing the patient to choose what products he will use to further promote better living.
Home Care Services – This service enables the Clinic to provide stoma and wound consultations and other services on an outpatient basis to enable the clinic to reach out to patients who have been discharged or are at home recovering.
Patient and Family Education – This includes lay fora and series of lectures that may help in educating the patient and their family about their current health situation. The clinic provides health guides that they can perform when they are at home, work, or in any of their daily activities.
Establishment of Support Group and Ostomy Association – These groups can be of great help to the patients. They can help patients resume their normal activities by going out, mingling, and spending time with other patients. They can also share each other’s experiences with regards to their stoma experience.
Wound and Pressure Ulcers Dressing – These services include minor to major wound dressing changes but without doing any sharp debridements, ranging from minor skin excoriations, burns, and to pressure ulcerations. If wound debridements are needed, the clinic will refer the patient to a wound physician on deck and the procedure will be done at the hospital’s operating room.
Ostomy Care and Pouch Application – This includes patient and relative participation in educating them to proper pouching techniques, stoma complication management and home setting situations.
The clinic uses a multi-parameter assessment when it comes to stoma and most especially for wound patients. A tool was developed to closely assess and re-assess patients, data are then documented, stored in separate folders including the patient’s progress on stoma and wound healing by the use of captured real time images with every patient visit. Consent is also obtained prior to every patient treatment.
1. General Services
After obtaining the patients consent, initial vital signs are obtained using blood pressure monitoring devices, obtaining baseline body temperature and initial assessment and patient data profile is obtained together with the initial photograph of the stoma and the wound.
Equipment is on standby to ventilate in cases where the patient experiences sudden difficulty in breathing including ambu bags, oxygen and suction machines.
Adjustable beds suitable for stoma and wound care procedures.
