  3.  Before availing of the promo, the patient must present their identification card or
    documents proving eligibility. Patients may present the following:
    • School ID
    • PRC ID
    • Certificate of Employment
    • Copy of Armed Forces of the Philippines Service Aptitude Test Accomplished Form with Stamp / Notice of Admission
    • Copy of NAPOLCOM Form - 1A / PNP Entrance Exam Accomplished Form with NAPOLCOM Stamp / Notice of Admission
  4. All patients eligible to avail the promo must undergo screening at TMCO-EVI first.
  5. The promotional offer is valid from August 15, 2024, to November 14, 2024.
  6. This promo cannot be used in conjunction with any other discount.
  7. For the purchase of goods and services which are on promotional discount, senior
    citizens or PWDs can avail of either the promotional discount or the discount provided
    under the Expanded Senior Citizen Act of 2010/An Act Expanding the Benefits and
